• Effectively communicating with multidisciplinary health care professionals such as home health skilled nursing, Physical Therapist, Occupational therapist, health care providers, medical service suppliers, pharmacies, and hospitals.• Coordinate patient care to include day to day calls for refills and questions or concerns to the provider for patients• Communicating effectively/ appropriately with patients, caregivers, and family members in regard to patient care.• Triaging calls for urgency and medical emergencies. Contacting/ notifying providers appropriately • Prioritizing task for myself and for providers• Giving verbal orders on behalf of providers when reasonable and within their guidance. • Documenting and generating documents using electronic health records• Faxing documentation to appropriate facilities to include providers paperwork and request for patient medical records using HIPPA as a guideline. • Detailed reporting and documentation of phone calls and all worked performed.• Create new orders and specialist orders to include STAT orders. • Maintaining confidentiality and privacy of patient information.• Taking initiative to perform task without being asked.
