While assigned as Director of Clinical Services at Horizons Home Health Services, I maintained several roles. Most important is leadership and supervision to the staff in the area of providing quality patient care. I prepared and conducted in-service meetings on a regular basis by presenting topics as delegated to me by the President of Horizon Home Health Services. I reviewed the clinical OASIS 485 and other associated paperwork along with completing necessary corrections. I was prepared to meet with the State of Michigan, CHAP, and CMS for surveys and other purposes. I made myself available during all hours by phone, email, and text to staff for any questions or concerns regarding their responsibilities.

**Laid off due to facility closing**Assigned as a Field Nurse to Interim Health Care, I fulfilled several responsibilities with the main focus on patient, family, and caregiver issues. I completed all required forms including the Medicare OASIS forms regarding outcomes and assessments. I carefully followed all directions and maintained accurate notes regarding start of care, recertification, follow-up, revisit notes, doctor’s orders, and discharge for all patients.I assessed patients, reviewed and listed medications, completed comprehensive teaching to the patient, family, and caregiver on diagnosis, processes, medications, and all other areas concerning their care. I collaborated with other staff members including Nurses (LPN), Home Health Aides (HHA), Social Workers, Physical Therapists, Occupational Therapists, Doctors and management to provide quality patient care. I transferred patients to the hospital as needed. For all patients, I effectively incorporated the Subjective, Objective, Assessment, Plan, Implement, Evaluate, and Revise Process. I supervised HHA’s and LPN’s, attended and actively participated in in-service and staff meetings, and complied with company policies, state and federal regulations as well.

Assigned as a Field Nurse, my primary responsibility was to provide quality patient care including providing assessments, listing of medications, teach patients, families and caregivers regarding diagnosis, self-care and medication issues. I attended and participated in in-service and staff meetings on a regular basis. Accurately provided documentation, including the OASIS format, visit notes conference notes, Doctor’s orders and other necessary records. I worked with multi-disciplinary team members to include Home Health Aides, Occupational Therapists, Physical Therapists, Social Workers, Doctors, and Nurses regarding patient care issues. I applied the Subjective, Objective, Assessment, Plan, Implementation, Evaluation, and Revision format to my work as a Field Nurse.

As a Charge Nurse for 15 – 30 residents at Cambridge South, I performed many duties including but not limited to administration of medications (IM, SQ, PO, inhalation, topical, PICC lines, Peg Tubes) and complete treatment to the residents. My supervisory responsibilities included managing CENA’s, overseeing assignments, breaks and ensured quality compassionate care was being given to the residents at all times. I was responsible for accurate documentation including the MAR, TAR, nursing reports, Medicare and PRN charting. I was proficient when managing emergencies successfully including 911 calls for CPR and other acute status changes. I attended and actively participated in scheduled staff and in-service meetings and maintained an almost perfect attendance. My dedication was demonstrated with my willingness to continue to work after the shift ended in order to complete required assignments and paperwork. I was always willing to work additional shifts or extra time when asked by supervisors. I maintained a professional, pleasant, helpful team work approach at all times. As a team player, I would assist other nurses with answering questions about clinical issues or with a resident transfers. I trained staff, family members, significant others and caretakers regarding resident care issues. Resident rounds were completed at the beginning, end, and throughout the shift to ensure the comfort, safety and general well being of the residents. Narcotic counts were completed at the beginning and end of shifts along with the ordering medications, as needed. The diagnostic entities included COPD, DM, CHF, CVA, Wheelchair bound residents, HTN pain management, diagnoses of residents for hospice care and oxygen therapy, as well as, psychological and emotional components. I executed physician orders as directed, completed various procedures including insertion of Foley Catheters and Straight Catheterizations and Suprapubic catheter changes.

While Assigned as a Charge Nurse At Greenfield Rehab and Nursing Center, I was responsible for 15 – 20 residents working primarily the afternoon shift. My duties included but was not limited to administration of medications and to complete treatments to the residents. I also was responsible for accurate documentation of the MAR's, TAR'S, Medicare and PRN charting and nursing reports. Included in the adult population were tracheostomy, hospice residents, including CHF, COPD, CVA, DM, HTN and other diagnoses. Supervision of the CENA's included completing their resident assignments, breaks / lunch times & being available to them at all times. I attended & actively participated in in-service & staff meetings and maintained no pattern of tardiness. Successfully managed emergencies which included 911 calls for CPR and other acute status changes. I was responsible to report with ongoing and oncoming shifts as well as a complete narcotic count and reordering of medications as needed. Resident rounds were completed at the beginning, end and throughout the shift to ensure the general well being, comfort, safety of the residents was being maintained. I spent several hours off the clock to complete shift work and paperwork after the shift ended. I would assist other nurses with answering questions and clinical issues or with resident transfers. I maintained a Coumadin therapy log on assigned residents. I trained residents, staff, family members, significant others and caretakers regarding resident care issues. I prepared for state surveys by participating in in-service practices and reviews of policies and procedure. I also would contact the physician on duty regarding resident care issues & executing their orders as directed . I completed various procedures as ordered such as Foley catheter insertions, Straight catheterizations, suprapubic catheter changes, and PICC line dressing changes.

While assigned at Bloomfield Orchard Villa, I assumed responsibility as a Charge Nurse working primarily the afternoon / midnight shifts responsible for 25-50 residents. I administered medications, PO, IM, SQ, PICC Line, Peg tubes ,topical, and inhalation, and completed treatments to the residents. I supervised up to 4 CENA’s including completing their resident assignments, break and lunch times. Responsibilities included overseeing CENA's work to ensure quality compassionate care was being given to the residents and offered to assist them as needed. I successfully managed emergencies including 911 calls for CPR and other acute status changes, sometimes contacting the doctor beforehand. Accurate documentation the MAR, TAR, nursing reports, Medicare & PRN charting. I attended and contributed to scheduled in-service and staff meetings and maintained an almost perfect attendance. I was also available to work extra shifts or time upon request of supervisors and willingly worked off the clock to complete shift work and paperwork. While employed on 2 separate occasions at Bortz Health Care at Green Lake, I was assigned as a Charge Nurse to 30-50 adult residents. My duties included, but not limited to medication administration, PO, IM, SQ, topical, inhalation with rare Peg Tube residents. I supervised CENA’s along with completing their resident assignments and their break and lunch times. I would oversee their work to ensure quality compassionate care was being given to the residents at all times. I attended and participated in in-service and staff meetings. As supervisors would ask, I was available to work extra time and shifts as needed. Accurate documentation included that of MAR’S, TAR’S, Nursing Reports, Medicare and PRN charting. Honorable Discharge from the US Navy with the rank of Lieutenant. Within the 7 years in the military, I acquired experience in various Registered Nurse positions. My experience included work within the Orthopedic Unit, Psychiatric Unit, Adult Intensive Care Unit (ICU), Postpartum Unit, Neo-Natal Intensive Care Unit (NICU) and Normal Newborn Nursery (NNN). I also have 3 years experience in outpatient assignments with Obstetrics/Gynocology (OB/GYN) clinics.
