I am responsible for completing evaluations on people in their own homes, and make recommendations for safety and types of skilled therapy that will be appropriate for them. I work closely with several OT Assistants and collaborate with all disciplines.

I complete evaluations and supervise 3 OT assistants needed for 2 nursing homes in the Hamilton, OH area. On weekends provide on-call services to 4 nursing homes.I will be working on a PRN basis for the East Side of Cincinnati.

I provided contract staffing help at different settings for certain length of times to cover vacations, leave of absences, and once to cover while a new graduate had been hired. She needed to take her boards, pass, and then go through orientation, so I assisted with covering patients while the hospital and manager addressed all of those issues.

Reported to Rehab Director. Supervised 2-3 client teams; administered recommended OT services to each team comprised of 50-55 clients each.• Earned spotless safety record from JCAHO through close collaboration with the Interdisciplinary Team on a weekly / daily basis via meetings and e-mail to identify and immediately address potential risks.• Visited and assessed approximately 260 client homes annually to identify and recommend action re safety and equipment issues; i.e. assembling tub seats and hand held showers and installing raised toilet seats and toilet safety frames • Successfully managed the Electronic Medical Record (EMR) transition from MediTech to Epic.• Updated up to seven treatment plans daily and coordinated with Restorative Aides when clients were discharged from active therapy.• Volunteered and served on Service Excellence Committee for two years; charged with improving staff morale in an ever changing and high volume work environment. Coordinated Employee of the Month Program as a key deliverable.

Hospital system comprised of one suburban and four rural hospitals, multiple outpatient facilities, 120 bed nursing home with 24 bed skilled nursing facility and an acute rehab center.Senior Clinician III OT • Combined knowledge, experience, team orientation and patient focus to adapt to OT needs of large hospital system; moving between hospitals and facilities to contribute to best possible patient outcomes.• Designed and implemented Student Programs at three hospitals; aligning senior students with field work opportunities.
