I have been a nurse for 23 years and am now the clinical nurse lead working with Coordinate My Care, or CMC. This is a “digital care pathway” that gives patients a voice to inform health professionals about the care that they would like to receive and where they wish to receive it. Digital/remote work has changed hugely especially since Covid. It has allowed us to help and inform patients in a variety of ways. and this will be a huge factor to help improve an NHS service that is struggling in this economical world.Working with CMC and talking to patients about the CMC Urgent Care plan has given me greater insight. It has made me realise how many patients want to have their preferences and wishes recorded and want these to be available for all health care professionals that are involved in their care. I see how technology has given what we do a whole other dimension, as CMC provides instantly available comprehensive information.Whatever nursing role I have worked in I was known, above all else, to be collaborative and supportive. This has served me well during my nursing career. The support and understanding I provide for my patients go hand in hand with the way I work with other health care professionals. I truly believe that these two factors are equally important to ensure that we can deliver the best care and service. Working collaboratively and supportively demands a sense of compassion and true team work and sometimes a need to drop the ego; it truly enhances care and treatment. CMC’s expansion around the UK is an exciting opportunity. I have experienced first-hand in real scenarios – both palliative and in emergency situations – the benefits of having a CMC urgent care plan. These benefits are experienced by patients and families and also health care professionals including the emergency services who depend on accessing patient information 24/7 to provide the best possible care then can provide. Ambulance crews are the first to tell you how valuable it is to be able to view a CMC Urgent Care plan at 02:00 in the morning and find what could be life and death information about the patient they are going to see.
