Provide regular support services to 32 families residing in subsidized apartments. Services include but are not limited to: assessment and referral for brief interventions, collaborative support for on-going recovery from substance use disorder, hosting family-oriented support groups and social gatherings, and referral to community-based mutual aid and/or community organizations.Conduct research, establish relationships, and collaborate with community-based organizations to provide trainings, workshops, and other services that will assist residents with concerns such as housing and food security, ADLs, and even professional skill building.

Conduct culturally-informed biopsychosocial assessments for adult clients who are seeking gender affirming care. Assess individual clients for gender dysphoria, as described in the DSM-5, and complete all related documentation, such as a ‘letter of support’ for gender affirming surgeries.Research and complete outreach to potential referral sources for clients who live in underserved areas, establishing a database that can be utilized by the multi-disciplinary team to better serve our clientele.

Maintain a caseload of 12 adult male clients in house of corrections drug and alcohol treatment program, providing individualized treatment such as assessment, diagnosis, treatment planning, and individual therapy sessions. Provide crisis intervention, where necessary, to aid in de-escalation of clients experiencing suicidal and/or homicidal ideations and/or self-injurious behaviors.Develop curricula and facilitate group therapy for 40 clients utilizing evidence-based practices such as cognitive behavioral therapy, rational emotive behavior therapy, and motivational interviewing. Advocate for the expansion of mental health substance use disorder services within the program, leading to the creation of a co-facilitated trauma-informed group for clients with co-occurring disorders.

Research and develop market-specific strategies for partnership with community-based organizations to reduce impact of opioid epidemic on customers/clients. Balance knowledge of evidence-based behavioral health interventions with company-specific data to advise pharmacy team in on-going partnership determinations.

Maintain a shared caseload of 12 young adult clients, offering individual case management services such as appointment management, resource brokering, and support for activities of daily living (ADLs). Provide crisis intervention for clients experiencing mental health crises including but not limited to: psychosis, manic episodes, suicidality, and self-injurious behaviors.Develop curricula for and facilitate daily psychoeducational groups for clients with an emphasis on ADLs, emotional well-being, and education/career support. Communicate with client and collaborate with the multi-disciplinary team to identify client goals and implement treatment plan objectives in day-to-day services.

Oversee a caseload of 12 adult clients in an inpatient treatment facility for clients with co-occurring substance use disorder and mental illness. Caseload specific treatment includes but is not limited to: assessment, diagnosis, treatment planning, individual therapy sessions, and case management for aftercare solutions. Provide crisis intervention, both in inpatient and detox level of care, to aid in de-escalation of clients facing difficulties in on-going sobriety, self-injurious behaviors, and/or altercations with other clients. Develop curricula for and facilitate psycho-educational, cognitive behavioral, developmental, and process group therapy to aid in clients’ on-going treatment plan goals. Attend regular multidisciplinary case management meetings to ensure that clients are transitioning appropriately through the continuum of care available to them. Advocate for and subsequent implementation of on-going policy restructuring to support systemically oppressed groups.
