Developed and disseminated a dementia and brain health education program for community members and gatekeeper organizations (i.e., agencies/community based organizations who serve older adults and can help identify those living with dementia). Collaborated with Duke University School of Nursing to provide dementia trainings to healthcare professionals and skills training to caregivers. Developed, implemented, and managed a Dementia Care Management Program for persons with dementia who live alone and ensure their ability to safely remain in their homes with appropriate home and community services. Supervised two Dementia Care Managers, dementia volunteers, and student interns. Prepared a semi-annual report to ACL every 6 months that detailed the grant activities during the reporting period and documented the unduplicated number of individuals served by the grant and the number of direct services hours provided. Monitored evaluation components and ensured key data points were collected and transmitted to the independent evaluator. Managed and co-facilitated the development of Durham's Master Aging Plan and their acceptance into the WHO/AARP's Network of Age-Friendly Communities. This included developing and presenting a proposal to the Durham County Board of Commissioners to fund the development of the Master Aging Plan.
