CurrentCatholic Hospice provides responsive, quality, end-of-life care to people of all ages in Broward and Miami Dade CountiesDeveloped, implemented and evaluated territory plans to achieve goals and initiate strategies and actions for a newly established territory in a market new to the organizationEstablished and maintained professional relationships with all referral sources, including physicians, hospital personnel, nursing homes and assisted living facilities to drive territoryadmissionsFacilitated contract bed agreements with 5 hospitals including North Broward Hospital District and 6 nursing homes in territory resulting in both census growth and increased admissions aswell as improved patient satisfactionRecruited 5 NEW physicians to become contracted with Catholic Hospice resulting in referral source loyalty and admission growth of 20% directly associated with these contracts Goal attainment 10 out of 12 months
