As Area Director of Case Management I coordinate the care of patients across the continuum for our LTAC patients by providing support, education and optimum utilization of resources. The coordination activities of the case management departments is multidisciplinary to provide safe and appropriate quality care in the most cost effective manner while ensuring each patient receives their maximum benefit of their health care services.

Travel position. Overseeing multiple case management departments within the Mid-South District. Coordination, education, training of new case managers. Implementation of Kindred policies and procedures for newly acquired Kindred facilities. Mentored individual LTAC Directors within the facilities to enhance the quality of patient management and satisfaction while promoting continuity of care and cost effectiveness through the integration of function of case management and utilization review.
