Through its programs in 22 states and the District of Columbia, AmeriChoice serves over 2 million beneficiaries of Medicaid, Children’s Health Insurance, Special Needs Plans and related government-sponsored health care programs. AmeriChoice health care programs are uniquely designed to address the complex medical and social needs of the populations it serves, especially the chronically ill, those with disabilities and others with higher risk medical, behavioral and social conditions. AmeriChoice focuses not only on its members’ medical issues, but also on the social, behavioral and economic barriers members face in improving or maintaining their health status. AmeriChoice coordinates resources among family, physicians, other health care providers, and government and community-based agencies and organizations to ensure continuous and effective care. The unique AmeriChoice Personal Care Model establishes an ongoing relationship with individuals who have serious and chronic health conditions to help them maintain the best possible health and functional status. AmeriChoice programs focus on high-prevalence and debilitating chronic illnesses such as hypertension and cardiovascular disease, asthma, sickle cell disease, diabetes, HIV/AIDS and high-risk pregnancies. Distinctive outreach and education programs, developed by leading researchers and clinicians, are used to target and intervene in the medical conditions most common among individuals served by AmeriChoice, as well as to ensure preventive care for children and adults.
