Strengthen and maintain partnerships with government and non-profit CoC organizations. Position TLC as a model of integrity and quality in serving vulnerable community members. Advocate for TLC programs within the CoC and publicly to maintain our role as a best-practice leader.Work with the Director of Operations on overlapping issues in case management and operations.Accountability and Ethics: Ensure accountability in TLC’s and partners' conduct, prioritizing guest safety, health, and dignity.Provide regular one-on-one supervision to all case managers to foster growth, guidance, and review of case plans. Ensure data quality, balance caseloads, monitor case manager well-being, and lead monthly case review meetings.Uphold ethical, equitable, and culturally respectful practices in case management, emphasizing the dignity of all individuals.Offer case management to TLC guests with high-intensity needs, including veterans and survivors of domestic violence.Provide de-escalation, mediation, and counseling for conflicts involving guests, volunteers, and staff. Conduct annual reviews for the case management team.Oversee guest records, ensure and maintain quality assurance for required HMIS data across all projects.Inter-Agency Collaboration: Lead case staffing and coordinate with CoC partners for By-Name List (BNL) meetings. Represent TLC’s case management efforts in OPEH and CoC director-level meetings.Oversee the Workforce Development Manager’s responsibilities, providing guidance on both workforce and case management aspects. Conduct regular one-on-one supervision to support decision-making and case management planning.Regularly assess program performance, identify growth opportunities, refine participant policies and processes, and develop employer relationships. Implement a system for tracking participant outcomes to meet City and County requirements.

Provides direct supervision for Homeless Outreach staff, overseeing the provision of homeless outreach services for unsheltered single adults served at Cornerstones.Oversees the outreach schedule of the Homeless Outreach team which engages people living in the community in places not meant for human habitation.Coordinates the Hypothermia Prevention Program and oversees service provision at the Hypothermia Prevention Program and drop-in day services at the Embry Rucker Community Shelter (ERCS).Oversees the Operation Stream Shield (OSS) program and visits work sites as needed.Provides direct supervision for QPID Case ManagersAssesses housing barriers of single adults who are experiencing homelessness to determine housing and service needs.Develops housing procurement, financial, and self-sufficiency case management plan with unsheltered individuals.Determines appropriateness of individual for receipt of rapid re-housing funds and recommends amount. Allocates and tracks use of rapid-rehousing funds including Emergency Solutions Grant (ESG) and Virginia Housing Solutions Program (VHSP).Assists in the development of programs and evaluation of all services provided, ensuring the services meet predetermined local, state, federal and agency outcomes.Maintains effective partnerships with Fairfax County Office to Prevention and End Homelessness (OPEH), the Fairfax County Health Department, the Fairfax County Community Services Board (CSB), Projects for Assistance in Transition from Homelessness (PATH), Department of Family Services (DFS), and the Fairfax County Department of Adult and Aging, to ensure clients receive holistic, integrated care management across Cornerstones.Maintains case and Homeless Management Information System (HMIS) files that meet agency and contractual standards. Serves as HMIS Super User.

- Responsible for working collaboratively with the multidisciplinary Medical Mobil Outreach Team to provide medical assistance to unsheltered individuals.- Responsible for developing and maintaining a schedule of regular street outreach stops including mental health drop-in centers, local soup kitchens, camp-sites, and other consumer gathering places.- Provide ongoing case management to unsheltered homeless individuals including client assessment, individualized service planning, and crisis intervention services as needed and advocate for access to mainstream medical, mental health, substance abuse, dental, public benefit, housing/shelter, employment, and other human/social services.- Provide direct oversight of all ERCS weekly drop-in activities and work collaboratively with the weekend outreach services coordinator to ensure overall comprehensive ERCS outreach activities.- Provide support, problem resolution and crisis intervention to residents.- Respond to community requests for assistance in working with people who are unsheltered.- Act as the case manager / point of contact for all outreach clients.- Maintain documentation of all pertinent interactions and contact with outreach clients.- Work closely with the Residential Manager to coordinate winter seasonal programming, services and staff.- Coordinate staff training, meal planning, laundry, transportation, supplies and all other facets of winter seasonal programming.- Responsible for maintaining up-to-date and accurate data in the Homeless Management Information System (HMIS).- Prepare reports and statistics related to all outreach efforts.
