Director Of Case Management
CurrentStrengthen and maintain partnerships with government and non-profit CoC organizations. Position TLC as a model of integrity and quality in serving vulnerable community members. Advocate for TLC programs within the CoC and publicly to maintain our role as a best-practice leader.Work with the Director of Operations on overlapping issues in case management and operations.Accountability and Ethics: Ensure accountability in TLC’s and partners' conduct, prioritizing guest safety, health, and dignity.Provide regular one-on-one supervision to all case managers to foster growth, guidance, and review of case plans. Ensure data quality, balance caseloads, monitor case manager well-being, and lead monthly case review meetings.Uphold ethical, equitable, and culturally respectful practices in case management, emphasizing the dignity of all individuals.Offer case management to TLC guests with high-intensity needs, including veterans and survivors of domestic violence.Provide de-escalation, mediation, and counseling for conflicts involving guests, volunteers, and staff. Conduct annual reviews for the case management team.Oversee guest records, ensure and maintain quality assurance for required HMIS data across all projects.Inter-Agency Collaboration: Lead case staffing and coordinate with CoC partners for By-Name List (BNL) meetings. Represent TLC’s case management efforts in OPEH and CoC director-level meetings.Oversee the Workforce Development Manager’s responsibilities, providing guidance on both workforce and case management aspects. Conduct regular one-on-one supervision to support decision-making and case management planning.Regularly assess program performance, identify growth opportunities, refine participant policies and processes, and develop employer relationships. Implement a system for tracking participant outcomes to meet City and County requirements.
