Practice Support Program fosters transformation in patient care by helping physicians build capacity in their practices through the use of coaching, practice facilitation, evidence base innovative clinical and practice management tools and strategies.Focusing on the Institute of Healthcare Improvement Triple Aim model - the experience of care, population health, and per capita cost.-Facilitated and coached family physicians, allied healthcare professionals, and MOAs/administrative staff to clinical workflow, procedures and processes using Electronic Medical Records (Wolf, Med Access, Oscar, Intrahealth), the Model for Continuous Improvement, Quality Improvement, and LEAN Six Sigma methodology.-Broad knowledge of clinical practice that includes comprehensive knowledge of population-based care and prevention using agreed-upon best practice quality improvement measures, guidelines, and protocols.-Knowledge of expanded chronic care model - Chronic Pain, Health Literacy, Adult/Child Mental Health, Advance Access, and End of Life.-Successfully coached/support primary care practice teams through the change management process.-Handled and supported account management assigned portfolio of geographical located clinics and physicians in VCH health authority-Demonstrated experience facilitating change, group process, and team building.-Demonstrated ability to function as an effective team member, leader, and facilitator.-Communicates effectively and in a compelling manner both verbally and in writing to a variety of audiences and stakeholders.-Demonstrated computer skills with various intermediate PowerPoint, Excel, word processing, database, and client management computer applications as well as various Internet browsers and computer office applications.
