As a Social Services agency specializing in Support Coordination, I focus on delivering essential services to adults diagnosed with Intellectual, Developmental, and Physical Disabilities, as well as Autism Spectrum Disorder, across Pennsylvania who are enrolled in the Department of Human Services Medicaid Waiver Program. Our dedicated Support Coordinators work closely with adults to assist them in navigating, comprehending, and accessing the required services to promote independent living within their homes and communities.In the role of Support Coordinator/Service Coordinator/Case Manager, you will engage in essential case management activities to address the needs of clients within the consolidated care team. This pivotal position involves crafting individualized care management plans, executing the care plan by coordinating services, and conducting thorough monitoring and evaluation of clinical outcomes to ensure the delivery of professional, comprehensive, and cost-effective services.
