As the Social Worker at Kasier Rehabilitation Center, I was responsible for all discharge planning including ordering home equipment, setting up home health or outpatient therapy services, arranging for admission to a skilled nursing facility, and setting up follow up doctor’s appointments. I was also responsible for attending the twice weekly team conference meetings and creating the team conference document on each patient. I also did all insurance communication including sending weekly updates to insurance companies to prove patients were making progress, asking insurance companies for additional authorized days, and working with the insurance company to find in-network outpatient services. In addition to this, I counseled families, connected them with community resources, helped them file FMLA paperwork, and addressed other pertinent needs.

Hospice offers dignified living at the end of life. I provided services such as socialization and companionship as well as legal aid and resources for those in need. Serving in rural communities challenged my knowledge of medical care in rural settings and forced me to find creative solutions to meet the needs of the patients.

As a social worker at Hillcrest, I worked with doctors, nurses, patients, and their families to set up a safe discharge plan. This includes referring to outside resources like nursing homes, home health, hospice, and medical equipment companies. I also attended daily staffing meetings to discuss each patient's status.
