• Admit patients to home health, record, and analyze patient’s medical histories, symptoms, and conditions.• Administer medications and treatments ordered per physician• Create patients care plan with certified nursing assistants and licensed practiced nurses and supervise their plan of treatments• Comply with procedures, regulations, and rules in maintaining medical records and Medicare regulations and guidelines• Teach patients and families about treatments and medications• Provide psychological support to patients and families• Coordinate and manage multi-disciplinary team evaluating and treating mental/substance abuse patients in the community

• Responsible for overseeing each patient’s overall health and medical histories• Supervise licensed practical nurses and other nursing staff to ensure quality care for residents at nursing home• Responsible for advanced activities such as starting intravenous infusions, administering oxygen, monitoring

• Admit patients to hospice and maintain the comfort and quality of life for patients diagnosed with a terminal illness• Communicate with patients and families about quality of life and changes in care plan or treatment• Assist coroner with medication disposal and care when patient expired• Supervise licensed practical nurses and certified nursing assistants per Medicare

• Assist patient and family during childbirth• Administer fluids, pain medications, and labor stimulating medications• Monitor vitals, strength and timing of contractions and cervical dilation• Assist physician with epidural administration and child birth process• Recognize complications and assist with emergency surgery and provide baby care after delivery
