Established partnership program between Somerville-Cambridge Elder Services and CHA Cambridge Hospital. Engaged hospitalized older adults at risk of readmission or a long-term stay at a skilled nursing facility, and created plans that allowed them return home to heal. Resided at the Maggie’s Place maternity shelter to provide 24-hour support for diverse community of pregnant and parenting mothers experiencing homelessness.
