    Broad Based Training is a 2-year program where trainee doctors rotate through 6 months in each of GP, Medicine, Psychiatry and Paediatrics. They are often choosing between two (or more) of these as a possible career, and this gives them time & experience to assist their career planning. When they exit into their chosen discipline, they get a year's credit for time spent in this BBT program, entering ST2 in their chosen discipline. Some trainees opt a completely different specialty, but they too benefit from the range of generalism encountered.There are 7 Health Boards in Scotland where this is run - Ayrshire & Arran, Dumfries & Galloway, Greater Glasgow & Clyde, Grampian, Highland, Lanarkshire and Tayside. This BBT program is also run in Northern Ireland (and I am happy to connect interested trainees with the TPD and team there).

    34 beds (2 HDU) Rural General Hospital, with an ITU 70 miles away in Inverness. I worked a 1-in-3 on-call pattern on weekdays, and 1-in-4 weekends (72+ hours: Fri 08:30 - Mon mid-morning). The catchment population is 22,500 (census data) but with summer & winter tourism, it is 60,000 for about 10 months of the year (coincidentally the population of Inverness, the nearest referral centre for tertiary care). There was no middle grade doctor (registrar) so trainees (max 3 years post qualification) escalated directly to consultant - which was a fantastic teaching and training opportunity for them. 14-16,000 A&E attendances, of which 1500-2000 will be children. There was no dedicated A&E doctor, so physician, surgeon (and anaesthetist) share this workload here, frequently jointly.Home to the highest mountain in the UK, Ben Nevis, and with 2 ski-resorts on our door step (Nevis Range and Glencoe) and lots of hills, it is hardly surprising that 80% of all mountain rescues in Scotland happen in our area, and feed into the hospital. Safe to say it was never boring!

In our rural location, we have to have a wide variety of general skills to ensure we can approach any medical or surgical problem, especially in an emergency. These students are almost at the end of their curriculum and are about to make the transition to being a qualified doctor. On their placement with us, we push them towards that role (with close supervision) - it is time for them to step up and start performing like the doctor that they are going to be. Feedback remains excellent.I was a regular examiner for the Final MB clinical OSCE.

They are exposed to a wide variety of general internal medicine, both acute presentations and chronic disease management. Students are often surprised at the variety of different conditions they are exposed to, and some of the rarities that we see, despite our small size and remote location.Our philosophy is that final year medical students should now be thinking and acting as if they were newly qualified doctors, not waiting until somebody hands them a scroll and congratulates them. So, rather than thinking "in this condition, we do 'x' as first line therapy", they should be thinking "I will consider 'x' and 'y' as possible diagnosis, and my initial investigations and plan will be 1,2,3..."I examined regularly for the Final MB OSCE examination for the University.

    From this post I gained leadership, management and team working skills. This post also highlighted to me, more than any before it, the value of my own skills & training as a professional. In terms of business skills, I left with an appreciation of how a billion dollar company maintains its market sector lead in a highly competitive industry.I was the sole physician for the M/S Jubilee. At capacity there were 2000 passengers & 700 crewmembers. My on-call commitment was 24-7 for the duration of the contract, the only respite being out with clinic hours if the ship was docked at port. I had a staff of three nurses in my department.This was the first time in my career where I was head of a department and ultimately responsible for those beneath me. In common with all crew, I was answerable to the Captain for Maritime discipline and safety at sea, but I could over-rule the Captain in medical emergency. I could ask for a medical opinion by the satellite phone, but I had to make the final decision. All Heads of Department attended fortnightly Captain’s Meetings. While on board, the company conducted a crew satisfaction survey, and the Infirmary staff rated highest of all.
