I prefer to challenge myself in my work life. I do not believe that it is possible to further professionalgoals without taking on new challenges and risks. I attended East Stroudsburg University, where I earned my degree in English Literature. Duringthe interim of college I was employed at Hemlock Farms Community Association in Lords Valley, PAas a Day Camp Counselor. About four months after graduation, I began full-time employment incustomer care service working at Rainbows of Learning in Newton, NJ in 2006, where I began workingas a Day Care worker. Later, I found myself employed in the Medical Profession field as a CommunityService Coordinator at Progressive Health of PA, Inc located in Milford, PA. The program focuses onthe care and inclusion of individuals with a traumatic brain injury.That position led to training and the eventual as a Certified Brain Injury Specialist/ Cognitive Rehabilitative Therapy Staff. The position allowed for one-on-one treatment to be establishedbetween the providing therapy staff and the individual. It allows for creativity in planning sessions,developing working relationships, and develop structured activities. I was also allotted the extra task ofmanaging an individual client's case. I was entrusted to work with that client on managing monthlyexpenditures, securing SNAP benefits, and SSI payments.Following my departure from working in TBI, I began to work at Access: Supports For Living in both their Middletown, NY and Newburgh, NY locations as a Health Home Care Manager. The job requires me to work with members to develop SMART goals to aid my members in achieving management of their Health Related Social Needs. I assist them with completing applications for various Benefits and entitlement programs. I also work closely with community support programs and help link my members to medical and mental health supports that they need to help monitor and improve the various medical and mental health conditions that our members are working to manage. I help to support engagement with their providers as well as connect to supports such as Peer Support, Housing, Substance Use Treatment, and food resources in the community. Most recently I have been assigned in our Health Home Plus program which works with members that are higher needs. Working with these members in a similar fashion but providing more support and guidance to promote their engagement. At times even assisting with transporting members to and from appointments to help with addressing major health and wellness needs.
