Our senior care RNs provide, initial evaluation and wellness review, comprehensive medication reconciliation and management, education, and med coordination between patient and their physicians. Whether living in a private residence, senior community or within an ALF; our flat rate in-home services are designed to relieve medication confusion and stress, encourage medication compliance and promote a team partnership between the family and all healthcare providers and professionals. If you have family members or friends residing in South Florida, full time or as "snowbirds", we're ready to help. We collaborate with home health agencies, case managers, guardians, caregivers and all those that truly care about out senior population. We're there when you can't be.....weekly reports provided to designated family members, caregivers and health care providers. All current and new medications are confirmed with patient physicians.
