Annie Pham is a health care professional committed to improving the health and well-being of under-resourced communities. She is passionate about primary care innovation and health equity, and as the Director of Social Health at Community Care Cooperative (C3), an accountable care organization (ACO) owned and operated by federally qualified health centers (FQHCs), she develops and executes projects to address the social drivers of health (SDOH). Her work entails capacity building initiatives to design workflows and equip care teams with the tools and resources required to identify and address unmet health related social needs (HRSNs) such as food insecurity and housing instability/homelessness. As part of a Medicaid Section 1115 Demonstration project in Massachusetts, she contributed to the design, implementation, and operations of a $25+ million portfolio of clinical-community partnerships and evidence-based nutrition and housing interventions, serving over 15,000+ people to date.
