- Conducting needs assessments for OAC program participants, developing individual care plans with participants, coordination of appropriate intervention and monitoring;- Providing advocacy, information and referral, crisis intervention;- Maintaining up-to-date case management files, including assessments, care plans and progress notes; compiling statistics;- Leading and facilitating support groups and workshops;- Assisting in the development and delivery of outreach strategies for seniors in the community, participating on community-based networks addressing Senior's and caregiver's issues;- Assisting with program planning initiatives, the on-going evaluation, development and maintenance of all aspects of the program;

- Works with the Adult Day Services Program group within the organization, where most clients serviced were diagnosed with Alzheimer’s or Dementia.- Plans, executes and evaluates daily activities, special events and celebrations for clients; observing and providing information for reports or proposals;- Promotes client’s mental alertness and independence through involvement in activities of interest (gardening, crafts, board games, stretching, etc).- Intervenes with participants in conflicts, using group dynamics techniques.- Observes and records clients’ functioning, self-care capacity, support, safety and service needs, the social circumstances of the client, the family and the caregiver, reporting changes to the coordinator; monitoring movement of clients at all times, ensuring they remain in the designated program areas;- Joins in-house committees and activities, providing any related assistance.
