Intensive Foster Care Case Manager
Current-Meet with children and youth at their foster home, school, or other community-based location weekly-Maintain communication with youth, foster families, and other treatment team members between meetings-Complete intakes and discharges promptly, including screening and assessments as well as individual action plan development-Identify children and youth’s needs and connect them to community resources to meet those needs -Complete regular reviews of progress toward goals to quantify child/youth functioning-Model skills for youth and coach/mentor them to develop their skills-Assist with internal and external reports-Actively participate in supervision and training as required-Ensure the safety and well-being of all youth by observing for signs of abuse and/or neglect-Provide services that respect each family and youth’s cultural background, heritage, and identity-Participate in emergency services rotation
