Responsible for creation of new patient and community engagement division that grew to 50 people. Leader in movement to identify, integrate and retain underserved populations for the health care system. Accomplishments included:✔ Completing major organizational outreach and education, including implementation of Affordable Care Act. Worked creatively with staff, peers and partners to expand outreach system in line with agency expanding its footprint outside the Hudson Valley.✔ Initiating and introducing The Patient Activation Measurement (PAM) program, which involved providing consumers with the knowledge, skill and confidence to take on the role of managing their health and healthcare. Tools utilized included administering thousands of patient self-assessment surveys and following up with coaching sessions.✔ Envisioning and implementing Local IMPACT to improve early identification and management of obesity, diabetes, hypertension, and stroke in communities of color. During the initial phases, we successfully engaged dozens of public worksites, food retail venues and community health workers. ✔ Overseeing AmeriCorps and other related community service initiatives, business development, fundraising, and all functions related to rapid expansion of the team
