  Under the direction of the Administrator of the County of Delaware, Adult & Family Services (AFS), I provide and coordinate Quality Improvement services within AFS including:• Monitoring for compliance of HUD federally funded programs that target chronically homeless persons and families who have behavioral healthcare disabilities and other needs.• Conducts agency site visits, file reviews, staff interviews, agency policies and procedures, grant management, service management and delivery, quality assurance and preparation of monitoring reports and provider follow-up.• Assists in the planning, monitoring and development of Quality Improvement processes for homeless programs and services through the Division of Adult and Family Services.• Collaborate and provide support to other Adult and Family Service staff as needed in regard to Quality Improvement activities relating to shelter programs, transitional and permanent housing programs that serve those with mental health disabilities.• Participates in homeless networking and coordination meetings including the Homeless Services Coalition Governing Board, County Advisory Team, Quarterly, and bi-weekly CoCAT meetings.• Establishes strong working relationships with other Delaware County offices and service providers.
