• Provide day-to-day support in alignment with client goals and support plans.• Mobility (using various hoists), Exercise, Personal Hygience Care, Catheter, Colostomy bag Management.• Ensure all client processes are accurately documented in accordance with organisational policy and standards.• Monitor levels of well being ensuring any concerns, incidents/accidents or near misses are reported and supported with appropriate documentation.• Practice Policies and Procedures, Focus on Clients Wellbeing and Rights.• Compliance with health and safety policies and procedures to maintain safe workplace.• Undertake other tasks as negotiated with the Service Delivery Manager.
