I was brought into the Maternity Team to lead on governance projects that the substantive team did not have the capacity to complete. This included:• Collating the evidence to close the backlog of open and overdue actions from serious incident investigations.o Where evidence was not available, I identified the tasks that needed to be undertaken to fulfil the obligations and ensured that they were carried out.o During my first two months in post, I closed almost 200 actions, 71% of the total, some of which were due in 2020.o By the end of my time in the department I had closed over 85% and had created a plan for the department to follow to close the remainder.• I also themed all serious incident, and root cause analysis, investigations undertaken by the maternity directorate since 2021 to identify areas to focus improvement.o This was carried out by reviewing all investigations, recording the patient demographics, and theming all care and/or service delivery issues.• I conducted a review of the serious incidents investigations related to settled claims, as part of the getting it right first time (GIRFT) initiative, to ensure that the team has correctly implemented the action plans.• I reviewed and analysed the Avoiding Term Admissions Into Neonatal units (ATAIN) data for presentations and board papers.• I assisted with requests for information from the integrated care board, regarding submitted serious incident investigation reports.• I also supported with the guidelines review group to ensure that there was no delay with the ratification of expiring guidelines and policies.
