With over a year of experience as a Population Health Clinical Pharmacist Fellow at the Avant Institute, I am committed to improving patient outcomes through effective medication management and education. I worked with two teams in Mount Holly, NC, to develop and implement population health programs that address the care gaps and needs of elderly patients with multiple chronic conditions. I also collaborated with physicians, nurses, and other healthcare professionals to provide optimal pharmaceutical care based on evidence-based practices.My passion for clinical pharmacy stems from my fascination with the intricate mechanisms of action behind medications and the scientific evidence that supports their use. I enjoy communicating complex information in a simple and engaging way that resonates with patients and audiences of diverse backgrounds and levels of health literacy. I believe that empowering patients with a deeper understanding of their medications not only enhances adherence, but also enables them to actively participate in their treatment decisions.
