Provide services (Planning, training, consulting, public speaking, advocacy) to families to help them develop a strategic plan that facilitates the navigation of school and community resources to change the trajectory of students who are at risk of school failure.

Assist in the development of plans, timelines, policies and procedures to ensure that set service standards related to quality and quantity are met.Serve as a liaison between the local school district staff, Vocational Rehabilitation staff and Parent to Parent to ensure families are provided culturally competent and coordinated care.Develop and foster positive working relationships with local school district staff and Vocational Rehabilitation staff.Provide follow up services that support appropriate grants/contracts, as indicated. Run, monitor corrections needed, and finalize reports detailing services provided by Transition Partners.Ensure that all necessary statistical and evaluation data is aggregated monthly into one system/program and analyze data to glean larger systematic patters and recurring issues.Identify, schedule, and organize appropriate training to support Transitions Partners in the school district.Conduct site visits to each district, as needed.meet and/or exceed minimum performance standards, as assigned.perform other duties as assigned.

A parent mentor is a parent of a student with a disability who currently receives or has received special education services through an IEP. The parent mentor is hired by his/her local school district to assist other parents with navigating home, school, and community. He/she brings the lived experiences that assist teachers and educators to understand the family side of the equation.
