Intensive Case Manager
CurrentI have worked in two programs at Sacred Heart, both as an Intensive Case Manager. The second position I have been in since November 2020, the Journey to Social Inclusion (J2SI) Program.As you can see from the video link, the J2SI program focuses on providing rapid access to housing, then supporting people to address issues that have had an impact on their well-being their after. I have supported our people by:- Completing applications to the NDIS- Referrals to allied health- Writing court support letters- Writing applications for funding and material aid to Queen's fund, The Resilience Project and Walter and Eliza Hall Trust- Creating a partnership with Jobs Victoria and Bunnings Warehouse- Develop support plans and goal setting- Conduct safety & risk assessments that protect both client and staff- Providing fortnightly supervision to staff- Implementation of peer support groups for staff whilst working from home during Covid-19 isolationThe opportunity to work with people for three years has given me an in depth understanding about the housing-first model that focuses on ending 'chronic homelessness'.
