Supportive Services For Veteran Families Case Manager
CurrentConducted outreach throughout Alabama counties by explaining the Supportive Service for Veteran Families (SSVF) program services, policy and procedures that resulted with over 500 low income veteran homeless and at risk of homelessness accessing assistance to secure or maintain stable housing annually. Conducted interviews and assessments in areas of housing, employment, behavioral health to develop a housing plan, referral sources and assessed skills required for successful tenancy. Applied evidence based interventions, including motivational interviewing and harm reduction to provided client centered service to veterans and develop and document individualized case plans. Provided case management as defined by best practices to establish ongoing relationships with minimum 25 veterans and/or veteran families monthly to reach identified goals with emphasizing transitioning into permanent housing. Submitted financial request for assistance based on veterans’ needs and SSVF policy guidelines. Documented and maintained accurate records for a minimum of 25 veteran families and information in HMIS data base as per data quality plan. Participated in team meeting and training to contribute to collaborative planning in case management and outreach. Maintained program weekly budget of $2450 to be used for housing and rapid rehousing for homeless and at risk veterans.
