• Plan and deliver Public Health projects in support of the Austin Black Pride mission• Procure sponsorship by establishing and fostering relationships with organizations including Gilead Sciences Inc., pharmaceutical company that develops ART and Prep amongst other antiviral drugs• Promote, coordinate, and actively participate in events on a local and national level that lead to meaningful educational experiences for BIPOC that are LGBTQ+ including Austin Pride Month and Austin Black Pride Week• Collection of community data surrounding patient experiences to enhance and personify initiatives• Facilitate initiatives to collect data from several populations across Texas including the LGBTQ+, BIPOC, HIV affected, transgender, health care, research, and ally communities

• Enhance patient outcomes through education, assessment of patient needs, planning and modification of treatment, and implementation of interventions• Connect underserved populations to education and treatment resources and provide peer support through shared experiences• Monitor intervention results and collaborate with cross-functional healthcare professionals for quality improvement• Develop and present cultural sensitivity training for patients who identify as LGBTQ+

• Develop, amend, and oversee action plans for PSS staff• Coordinate referrals and prior authorizations• Enhance access to internal services through management of nursing, therapy, and chaplaincy services, and assessment of patient needs

• Serve as a patient liaison and maintain reception• Coordinate of appointments• Coordinate patient referrals to other specialty clinics

• Conduct at home patient assessments• Develop treatment and action plans to prevent re-hospitalization• Data entry• Setup and configure EMR systems for patient populations in New York and Rhode Island and members of the Texas Star Plan
