Lead the Way Support Coordination offers both Support Coordination and Specialist Support Coordination tailored specifically to meet your unique goals. Our areas of expertise is in the housing sector and psychosocial disability. We are experienced in providing complex case coordination to mitigate barriers to plan implementation by interfacing between multiple sectors to achieve participants goals. This includes hospital discharge, release planning from incarceration and exploring housing.

In my consultancy role with The Duke of Edinburgh Award, my focus was to collaborate with Award Centre's to initiate program commencement, Online Record Book and program support and to guide new Award Centre's through the registration process and establish training and program outlines.

• Ensuring client engagement in a comprehensive holistic screening and risk assessment of complex relationship and social issues utilising the Detection of Overall Risk Screening (DOORS) • Providing case work support to a prescribed number of young people through face-to-face appointments including home visits and community meetings• Providing accurate written records of casework meetings in the RASA client management system and regular government reporting system• Regular liason with external stakeholders to ensure coordinated case management that promotes client wellbeing and learning outcomes • Planning and implementing projects as required • Supervising the operational work of allocated team members involved in delivering services and resolving issues• Providing advice and assistance to team members, with support for case management of clients assessed as the most complex • Providing clinical and operational supervision to case managers• Facilitation and coordination of The Duke of Edinburgh Award across 4 sites
