Admissions Skills• Exceptional at coordinating hospital discharges, admissions and external admissions as a member of a three person team that facilitates an average of 500 admissions per year.• Ability to work closely in coordinating and communicating with social workers, nurses and Directors of Nursing, related medical staff and family members.• Meet with families to facilitate smooth transitions into long term care, rehabilitation and assisted living services.• Excellent and accurate with documentation to include obtaining financial documents for financial approval for external admissions; generating billing numbers; meeting with families to obtain signatures for legal documentation; depositing and maintaining payments; executing unit releases.• Dependable census manager, to include reporting on a quarterly basis to the State of Maryland Department of Health Care Commission Office, maintaining daily census, and reporting to the Health Care Counselor.• Community Outreach. Host and facilitate monthly events . Conduct tours for Assisted Living, Long Term Care and Rehab Facility . Make announcements on local Television for our events and updates. Insurance verifications
