     Part of a team that created and designed the Emergency Housing Program pilot program for the City of Issaquah. Coordinated directly with the Behavioral Health & Homeless Outreach Program to identify eligible persons for the program, coordinated referrals into the program, and to coordinated services for persons referred. Worked individually with persons enrolled in the program to identify & implement specific goals created at program entry for each social determinants of health: personal, social & community integration, health & healthcare access, education, career & economic stability. Conduct intake assessments and review program rules, expectations, transition, and exit criteria. Collaborated with City staff to finalize and implement program services for each person enrolled in the program to include access to services, attendance, and progress towards goals.  Collaborated with other local area service providers in supporting individuals in the program. Conducted weekly assessment of progress in services, ensure measurable progress towards established goals, and adjust service provision based on unique needs of the persons served. Tracked data and progress towards goals and ensure successful achievement of program outcomes. Participated in case consultations and program evaluation meetings.  Prepared, submitted and presented written materials and reports for a variety of audiences, including service providers, supervisors, Department Directors, Boards & Commissions, Mayor, or City Council.  Participated in program development and ongoing program evaluation. Developed metrics to measure the effectiveness of programs and compliance with funding requirements.  Acted as integral part of the team and help design, implement, maintain, and evaluate all aspects of this program, consistent with Division, Department and City goals, and in compliance with applicable regulations and laws.
