With over four years of work experience in the healthcare sector and over 10 years of non-profit community engagement work, I have developed a deep passion for addressing social determinants of health (SDOH) needs and improving health equity outcomes for underserved populations. As the Social Needs & Partnerships Manager at People's Community Clinic, I lead the clinic's SDOH work and implement patient screening clinic wide. I also manage community partnerships with other community-based organizations (CBOs) and maintain healthy relationships with key stakeholders to ensure patients receive SDOH needs. I value the clinic's mission of providing high-quality, affordable, and compassionate health care to all, and I bring diverse perspectives and experiences to the team as an ethnic studies graduate and a former community organizer.In my current role, I have successfully launched and scaled the SDOH screening tool across the clinic, reaching over 20,000 patients and identifying their social needs, such as food insecurity, housing instability, transportation, and legal issues. I have also established and nurtured partnerships with over fifty CBOs, facilitating referrals and follow-ups for patients with SDOH needs. Additionally, I have developed and delivered trainings and presentations on SDOH best practices, data collection, and evaluation for clinic staff, partner organizations, and funders. Through my work, I have contributed to the clinic's vision of creating a healthy and empowered community.
