Develop and maintain productive working relationships with Managed Care Organizations/Behavioral Health Organizations to assure compliance with contract requirements to improve member outcomes.Oversee process for identifying and onboarding new Care Management Agencies in the Provider Network, develop and maintain productive working relationships with the Care Management Agencies to resolve issues and achieve high customer satisfaction with HHUNY. Collaborate with the Marketing Team to identify and develop Referral Sources for the Adult and Children’s Health Homes of Upstate New York by providing presentations to increase the knowledge and understanding of Community Providers regarding the benefits of Health Homes Care Coordination Services. Represent HHUNY/CHHUNY interests at the Office of Mental Health, Regional Planning Consortium-Western Region. Participate in the implementation of the various Performing Provider System Projects through the New York State Delivery System Reform Incentive Payment (DSRIP) Programs. Promote and educate community stake holders on benefits of Health Home Care Management.

Provide overall program management for Health Homes of Upstate New York (HHUNY) to assure that operating practices meet the NYS Health Home requirements and are carried out in a cost-effective fashion. Assure that administrative needs of the downstream care management agencies are met so that they may deliver timely and effective care management services

Produced a master project plan for implementing Phase II and Phase III Health Homes in a multi agency collaborative. Coordinated a survey of Provider Network health information technology capabilities/needs. Initiated policy and procedures for the collabortive, Health Homes of Upstate New York and consulted with the Care Management Workgroup on establishing practice standards and outcome measures.
