 I am an experienced consultant to healthcare systems, hospitals, hospices, home health agencies and innovative new senior access disease management companies to develop and implement Palliative Care, end-of-life and advance care planning programs. For nearly 12 years I consulted with Dignity Health providing development guidance for 36 new palliative care inpatient programs in hospitals in three states and continuous clinical oversight to the palliative care coordinators. Participated in quality improvement initiatives and co-led implementation of palliative integrative care bundle in the ICU in the healthcare system. Developed training manual for disease management company to train nurses working with cancer patients to proactively engage in end of life and advance care planning conversations and educated nurses and trainers. Collaborated with Kenyatta Hospital's Palliative Care program in Kenya to develop support group for parents of children with cancer and arranged for medicines and supplies to be donated from US organization for cancer population.Significant Achievements:-Advance care planning initiative rose from system-wide baseline of 15% to 85% in 3 years and maintained that level for 8 years it was monitored.-Advance care planning for nurses in disease management received national recognition-Presented twice at AAHPM on palliative care model and advance care planning model-Rural hospital with newly designed palliative care program recognized significant cost savings and reduction in readmission and mortality rate in first year.
