• Represents the agency in activities involving professional relationships with physicians, hospitals, public health agencies, nurses' associations, state and county medical societies• Serve as a public awareness representative for the agency, and am responsible for public education relative to home health care services available through the agency and payer sources and methodologies for obtaining such services• Responsible for maintaining and growing the Company’s client base through strategic sales planning and marketing• Serve as a liaison between hospital and nursing facility discharge planners by visiting patients, as requested, to ensure a smooth transition to the patients’ home• Participate in professional associations and organizations to enhance knowledge of the home health care industry and to understand needs of referral sources and patients• Establish and grow territory to meet established sales goals through effective communication of company offerings and initiatives
