• Provide strengths-based intensive in-home family therapy (IHT)• Engage identified clients and families with insight, behavioral, structural family and skills-based interventions where appropriate (ex. attachment-based psychodynamics, CBT and DBT) • CANS certified assessor via MassHealth (Medicaid); complete comprehensive assessment with diagnostic formulation + IAP (treatment planning) + CANS (multi-domain needs and strengths ratings instrument) for each identified client and family • “Hub” role; assess need for and manage additional CBHI and other community-based services like Therapeutic Mentoring and behavioral services• Insurance management (reauthorizations, etc.)

• Provide crisis counseling and therapeutic support focused on acute stabilization from a solutions focused perspective for disabled clients with issues ranging from severe mental illness to bereavement • Lead and co-lead daily groups integrating elements of CBT, DBT (ex. mindfulness & emotional regulation), interpersonal psychodynamics, skills-training (ex. “living in new housing”) and strengths-perspective (~6-8 daily group attendees)• Complete client assessments including daily mental status exams, treatment planning & goal setting, diagnostics and collecting client biopsychosocial histories. (Unit capacity of 12 clients) • Case management; medical (ex. connecting clients with PCPs, psych and other community providers), housing resources (ex. St. Francis House, sober houses, “partial plus”) and on-going treatment consultation and referral (ex. partial hospitalization, rest homes, skilled nursing)• Embedded within a strong interdisciplinary team including physicians, nurse practitioners, nurses and social workers in a hospital setting
