Care Coordination for Children (CC4C) is an at-risk population management program that serves children from birth to 5 years of age who meet certain risk criteria. The main goals of the program are to improve health outcomes and reduce costs for enrolled children.•Case manage pediatric patients that are at risk for developmental delays or disabilities, children exposed to severe stress in early childhood, NICU babies, and children in foster care•Perform Comprehensive Health Assessments including the Life Skills Progression and Ages and Stages Questionnaire (ASQ) screening tool to identify a child's needs•Act as a liaison between the parent/guardian and child's medical home•Assist with communicating with doctors, making appointments, exploring child care options, or making referrals to other agencies and programs•Provide patient teaching and education regarding age-appropriate growth and development and various childhood medical conditions•Perform contacts by phone, mail, home visits, meeting the child at medical home visits, or other locations that are convenient for the family

•Provide emergent nursing care to geriatric, adult, pediatric, mental health, and maternity patients with acute and chronic health conditions•Initiate patient assessment to determine immediate care needs•Collect information on patients•Provide care to patients with minor injuries and health issues •Act as a liaison and patient advocate for patients in collaboration with other members of the healthcare team to ensure patient’s needs are met•Multi-task in order to provide patient- centered care in a timely manner. •Educate patients and family members on various disease processes and health conditions•Determine the standards of care as needed to develop care plans•Perform nursing interventions as required•Act as a team member/leader to perform ER task efficiently through delegation•Continuously look for opportunities for professional development and improvement
