Project FacilitatorBlue Mountains Gp Network Sep 2009 - Jun 2012• Worked with the BMGPN consumer reference group and the connected health professional group to inform the development of this project. • Produced local directory of self management groups and activities including current use of one to one self-management by general practitioners. • Raised consumer awareness of CDSM and local self management initiatives. • Raised CDSM awareness amongst health professionals and increased referral to local self-management initiatives and health services by working with the General Practitioner membership of the Blue Mountains GP network, health professionals and local pharmacists. • Identified access issues and barriers for the local community to participate in CDSM. • Implemented in the local area “Moving On: a Self-management Course for people with a Chronic Illness” which is a generic self-management course designed to increase people’s knowledge and skills to enable them to self-manage. • Co-ordinated the evaluation of the whole project including qualitative evaluation of the local implementation of “Moving on”. (The actual evaluation was conducted by the Menzies Centre for Health Policy). • Worked with the Blue Mountains Aboriginal Health Coalition and "Healthy For Life Program" to pilot the Moving On program for the local Aboriginal Community. • Built on local activities to increase the long-term sustainability of local CDSM.
