Case Manager Navigator
CurrentCollaborate within a multidisciplinary team to engage and provide services to individuals experiencing homelessness.Build effective, trusting relationships with clients to facilitate access to necessary services.Partner with community health providers, businesses, and social service/housing organizations to reduce homelessness in targeted areas.Work collaboratively to address barriers to permanent housing, including obtaining identification, accessing legal aid, securing benefits, and developing life skills.Conduct initial client engagement, including needs assessment and triage planning.Assist clients in gathering required documentation for housing applications.Ensure clients are connected to all benefits for which they are eligible.Refer clients to appropriate services, such as healthcare, mental health support, and interim housing solutions.Identify housing opportunities and work to transition individuals into stable housing as quickly as possible.Maintain positive relationships with landlords and housing developers.Provide case management, aftercare, and follow-up services to clients who have been placed in permanent housing.Maintain accurate electronic records, client files, and statistical data.Use harm reduction and housing-first principles to guide client interactions and service planning.Collaborate as part of a team to identify and coordinate services tailored to the unique physical and cognitive needs of each individual.
