This is a new position within this organization, providing care coordination for individuals and families living with severe and persistent mental health disorders. (I’m assigned to nearly 50 units). The residents are referred by the County’s Behavioral Health department, and their apartments are located in a LIHTC multi family property and a 24 unit property with all subsidized units. Many of the residents have been living at the properties for years, some over 10 years! It’s been awesome to see how transformative high quality affordable housing is for people who had previously experienced chronic homelessness. The property managers live on-site which, IMHO, contributes to more effective communication with tenants, their neighbors. Housing with services IS the solution.I’m bringing decades of life experience and knowledge gained from working with some of the most innovative and impactful affordable housing and economic development programs in the country. In hindsight, I see how fortunate I was to participate in the training and professional development opportunities offered by those organizations. More importantly, how lucky I have been to work alongside dynamic, pragmatic, and compassionate colleagues and tenants to build communities of care and establish industry wide best practices. I am grateful to have this opportunity to work as a case manager with this team and these residents. I am looking forward to their guidance and collaboration in this role.

- Certified Rent Well Instructor (see rentwell.org) - Update and create Renter's Rights Guides hosted on SETA's website to provide public information about Oregon Tenant/Landlord laws and accessing rental housing in language accessible to people with at least an 8th grade education- Develop and present training workshops for staff and program participants on requested topics related to landlord/tenant law for community partner agencies- Facilitate monthly meetings with Housing Navigators representing over 20 partner agencies in order to share resources, best practices, and program updates- Coordinate with peers working for Lane County government and City of Eugene to leverage resources in order to provide ongoing technical assistance and professional development opportunities for front-line workers serving people experiencing housing instability- Utilize Meta Business Suite and Adobe Express to create and schedule social media posts- Utilize Google Workspace, Zoom, and Nationbuilder to perform other job duties

I was responsible for oversight of the day-to-day operations of the St Vincent de Paul Youth House, an innovative housing program providing female-identifying youth ages 16-20 with supportive housing. - Developed job description, policies and procedures for residents and staff - Facilitated weekly supervision meetings with team members - Collaborated with agency housing department, Youth House team, volunteers, and residents to create a nurturing environment supportive of residents' personal growth and academic success- Responsible for working with agency IT department, maintenance and other vendors to schedule and complete building projects- Coordinated with community partners to facilitate wrap-around services for residents who were participating in their programs - Participated in fundraising activities, including speaking to potential and current donors and contributing to grant applications- Active participant in several monthly and quarterly networking meetings, and developed productive working relationships with probation officers with Lane County Youth Services, McKinney-Vento workers from local school districts, and other organizations serving female-identifying youth ages 16-20 experiencing housing instability- Developed internship/volunteer position descriptions, recruited and supervised two Denver School of Social Work MSW interns, an Americorps VISTA member, and a University of Oregon intern- Acted as on-site property manager; conducting inspections, coordinating move-in and move-outs, conducting orientations to assist residents with understanding their rental agreement, issuing lease violations, and developing educational or practical activities residents could complete to "cure" or resolve lease violations.- Utilized Zoom and Microsoft Teams and managed multiple Outlook calendars- Utilized the federal HMIS (Homeless Management Information System) database to provide time-sensitive data and generate reports

- Provided on-going emotional support, resource referrals, and financial coaching services for young adult, first-time tenants living in affordable housing at the Polk Apartments- Worked closely with Polk Apartment housing applicants (mostly youth aging out of foster care system and/or experiencing housing instability) to obtain documents and to complete and submit applications- Acted as liaison with property management staff to assist residents with effective communication and abiding by rental agreement and community expectations- Met weekly with property management staff to proactively resolve tenant issues, planning and prepping residents for inspections, and collaborating on creative ways to resolve lease violations - Conducted regional resource mapping and developed a service provider directory of youth/young adult specific resources for employment services, continuing education, transportation, financial resources, etc- Developed positive, productive relationships with youth through extensive outreach to drop-in service centers, alternative learning sites, high schools, and NEDCO financial classes - Revised NEDCO’s Financial Foundations course to be relevant and engaging for youth ages 16-24. - Provided financial coaching to over 30 young adults who completed the 8 hour youth Financial Foundations course- Developed 1 hour workshops for youth participants in federal Workforce (WIOA) programs on various topics including; paying for college, budgeting, building credit, doing taxes, etc. - Participated in monthly networking and collaborative meetings with community partners- Utilized social media for outreach to young adults - Utilized Microsoft Office programs and hard copy files to organize participants' information and maintain confidentiality

- Worked with a multi-disciplinary team to provide trauma informed educational and workforce development programming for around 35 (new students per year) at-risk or “opportunity” youth age 16-24- Developed an alumni group of graduates to maintain community and connection by utilizing social media and hosting regular events to encourage Post Secondary Education access (such as "scholarship parties")- Provided on-campus support to AYW alumni enrolled in college courses by assisting students with identifying and utilizing school resources, effective communication with professors, and utilizing Google Workspace to organize documents and manage their time and personal finances- Developed and implemented a City of Austin internship program, placing ten interns in positions with various COA departments and local businesses - Developed and implemented post-secondary education and training preparedness programming, including; teaching classes about financial aid, grants, and scholarships; conducting career exploration activities with students, recruiting speakers and volunteers from different fields; coordinating field trips to local colleges and apprenticeships programs; assisting students with identifying majors and applying for college and grants- Coordinated with Austin Community College staff and faculty to create classes in Automotive and Welding for AYW students completing their GED or HS Diploma, working with faculty to evaluate and improve class outcomes - Developed and taught Financial Literacy curriculum - Field Supervisor for five MSW interns from the University of Texas providing case management and supportive services to youth - Facilitated small groups around personal growth topics as requested by program participants- Advocated for youth involved with the Municipal and Criminal courts, resulting in dismissed charges and reductions in fines due to student achievement- Responsible for monthly reports on program participation and outcomes

- Met weekly with students to facilitate entrance in college level classes, including; applying for financial aid, completing application process to Austin Community College and determining which Associates/Certificates to pursue- Provided job search assistance and referrals to community resources for basic needs- Coordinated with guest speakers to provide workshops on various topics.

- Developed and implemented a Resident Services program for Spring Terrace, a permanent supportive housing project offering 142 studio apartments for people who have experienced chronic homelessness - Responsible for operating an on-site food pantry, coordinating partner agencies providing on-site social and educational services, and obtaining resources for resident social activities- Collaborated with residents to establish a Resident Council that helped plan on-site activities to foster connection and organized tenants to advocate for themselves- Met weekly with Property Management staff to support resident retention by establishing best practices in communicating with residents, identifying ways residents could resolve lease violations, and addressing repeat behaviors- Provided individual case management services for 120 residents, including assistance with applications for benefits, transportation to medical appointments, and obtaining rental assistance and other basic needs

- Responsible for maintaining resident files and providing HUD required statistics and reports through various databases - Developed positive, supportive working relationships with families living in transitional housing and provided case management services including conducting needs assessments, determining eligibility, and establishing work plans to support them in achieving housing stability and educational and vocational goals- Assisted with navigating access to resources provided by DHS, the VA, Worksource, Social Security Administration, Victims Services, and affordable or subsidized housing programs- Provided group and individual assistance with creating resumes, career exploration, credit repair, and accessing funding for post-secondary education

- Responsible for maintaining resident files and providing HUD required statistics and reports through various databases - Developed positive relationships with families living in emergency shelter provided by a network of religious organizations, conducting needs and eligibility assessments in order to support them in achieving housing stability and educational and vocational goals- Accompanied program participants to appointments with various providers including DHS, the VA, Worksource, Social Security Administration, Victims Services, and affordable or subsidized housing programs

- Provided representative payee and case management services to 35+ individuals with a range of social barriers including; chronic homelessness, untreated mental and substance abuse disorders, developmental disabilities, domestic violence issues, and legal and financial histories.- Collaborated with clients to develop monthly budgets, made sure their bills were paid, and disbursed weekly checks with leftover funds- Worked with a small team of colleagues to provide services and maintain a safe environment in a day time multi-service center for people experiencing literal homelessness in the Bay Area- Responsible for maintaining client files and providing HUD required statistics and reports through various databases.

-Provided case management services for caseload of up to 15 families living in emergency shelter who were heavily impacted by the financial and emotional effects of substance abuse- Collaborated with program staff to maintain a safe and supportive environment for participants- Coordinated with community partners to provide services and additional resources for familiy members and children

- Supervised people participating in court-ordered residential treatment and documented their attendance in program activities- Drove a 15 passenger van with program participants to various community events- Provided coverage and supported participants in completing daily activities in all the residential units: Detox, Women's, and Men's- Completed an internship in the girls' Adolescent residential unit for my Bachelor of Social Work field requirement- Organized a fundraiser to raise $700 to purchase art supplies for the Adolescent programs
