 Connect families of the primary care office with desired and appropriate resources within the community-Connect and assist families with the reinstatement of healthcare coverage-Assure that the healthcare of the primary care patients are being met by assisting families with connection of the appropriate services and care coordination-Provide all necessary and requested referrals for children and families and follow up whether those services were secured.

- Provides receptionist support including answering telephone, greeting visitors, and responding to the needs of patients via call lights, and ensuring that request are either met or handed off to the appropriate nursing staff. -Ensures that health information records are properly obtained, duplicated, assembled maintained and disassembled. Inputs information into various computer systems. Navigates through various components of the computerized order entry and documentation system. -Coordinate charts and medical data productions, triage reception functions and act as emergency communication coordinator. Provide initial contact for the Critical Care Support team external to Emergency Department and interacts with patients and families.

-Provide supports coordination and resources for individuals with intellectual disabilities-To offer other forms of supports to individuals with intellectual disabilities aside from the financial aspect.

-Provide and assure the safety and well being of a child that may be in danger of abuse and neglect-Provide clients with resources and services to ensure a safe and healthy living environment
