My second year assignment involved working on Diabetes Prevention and Control activities within the CDC State Public Health Actions "1305" grant. My major activities included: partnering with organizations to increase referrals to Diabetes Self-Management Education Programs (DSME) and National Diabetes Prevention Programs (NDPP), working on a statewide needs assessment for diabetes educators, and conducting an environmental scan to create a payor matrix of health insurance providers who reimburse for DSME and NDPP.
