Case Management - Executive
Current• Case Management - Perform needs assessment of community dwelling seniors- Development of care plan- Facilitate timely & cost appropriate service referral- Coordinate care with internal & external providers- Review care plans & participate in case management discussions internally and with external partners • Work with the AAC team to design, implement activities and wellness programmes that focuses on building the elderly’s self-help capacity• Administration - assist the Manager- Design, implement activities and wellness programmes that focuses on building elderly’s self-help capacity including- Ensure compliance to Finance and Human Resource Policies, maintaining documentation for audits facilitated by Corporate Services- Prepare data for submission to MOH and AIC
