Professional Service Coordinator
Cultivate and maintain relationships with referral sources that include physicians and their staff and hospital discharge planners and social workers.Responsible for ensuring continuity between the hospital or community physician practice and the home setting in order to maximize patient satisfaction of home health services to optimize patient safety, comfort, and well-being.Improve awareness and confidence among healthcare professionals, physicians and patients regarding current home health options and capabilities.Coordinate referrals to ensure timely admission and appropriate patient care based on doctor’s orders. Maintain a current working knowledge of the Medicare regulations and guidelines.Understand and effectively communicate federal and state regulations, which affect the home care patient.Understand what is required by JCAHO for a patient to be admitted and discharged from a home care Agency.
