Care Coordinator
CurrentØ Ensure household mapping and screening of COVID-19 patients and their family members for mental health.Ø Community engagement to generate awareness on COVID-19 and mental health.Ø Responsible for supporting patients through COVID-19 diagnosis, treatment, and recovery with empathetic care and service delivery.Ø Facilitate early diagnosis and referrals for patients through active case-finding and contact tracing.Ø Ensure all diagnosed COVID-19 patients are initiated on treatment through variousØ Manage patient prescriptions and medication adherence including utilization of digital adherence technologies and other monitoring methods.Ø Identify high-risk patients and initiate referral protocols (patients with adverse drug reactions, mental health challenges, alcohol & substance abuse, etc.)Ø Facilitate access to government services such as nutrition support and other social support schemes.Ø Coordinate across public/private sectors, providers, and staff as required to manage patient movement/migration.Ø Report information accurately as required into data systems.Ø Extend support to various field events scheduled during the project period.Ø Report daily field activities to the Regional Coordinator.Ø Identification and engagement of Providers.Ø Coordination with program administrators and local health administration.Ø Coordinate with CCC and MIS for quality data collection and analysis.Ø Follow-up with engaged providers and FLWS for project mandatory services.Ø Manage project deliverables within stipulated deadlines and monitor engagement activities with private and public sector.Ø Ensure high visibility of the project through advocacy, networking and regular dissemination of information about the project activities.Ø Ensure confidentiality and security of the program data and information.Ø Perform other relevant duties aligned with the project objectives as required by the WHP.
