Provides case management, education, resource development, supportive services, and monitoring to families for one year in the Aftercare services program. Provid intensive in-home services, crises intervention, and on call on a 24-hour basis. Provid resource education and serve as a liaison with community services. Maintain clients’ records. Document all client contact, service providers contact, attempted contact, client logs, and all associated documentation. Complete safety assessments, risk assessments, and other assessments on a routine basis. Complete court reports and attend court hearings. Communicate effectively with clients, service providers, colleagues, and other community members by oral communication, e-mails, letters, reports, and in group settings.
