Provide care management for Humana members after a hospital or sub-acute discharge with goal to prevent unnecessary re-hospitalizations during the immediate post-discharge phase and to help them understand self-care requirements and be able to implement them and follow plan of care. Also, provide weekly care management services to Members who have had two or more hospitalizations in the past six months and have functional deficits with their ADL's to provide ongoing education to them to prevent unnecessary hospitalizations and ER visits and to help to maintain them at home.

Provide supportive and medical social services to terminally ill patients and their families in accordance with plan of care. Responsible for the development, implementation, and management of psycho-social services provided to hospice patients and families in the home care and nursing home setting. Provide psychosocial education to patients and family caregivers about coping skills, hospice and palliative care philosophy as well as nonpharmacological symptom management strategies. Assist with discharge planning, coordinating care, and helping clients navigate systems. Facilitate advance care planning and mediating conflicts within families, between clients and the interdisciplinary team and participate in interdisciplinary team meetings, care planning, and ethics consultations.
