As a certified geriatric pharmacist and certified Medication Therapy Management (MTM) provider, I believe every patient should have personalized regimen as well as the adverse drug reactions they will face. Therefore, to educate patients about their specific medication needs is very important. MTM service is a good resource to provide patient education, prevent adverse drug reactions, ensure drug safety (screen drug/drug, drug/diet, drug/diseases interactions), and enhance the drug therapy effectiveness. As a healthcare professional, I believe that illnesses create a vicious cycle that will bring down the economy and qualify of life of most people.
